That way, you are provided with a claim number and your insurance adjusters contact information immediately. If you are involved in an automobile accident involving injuries, you should notify your insurance company instantaneously and make sure the accident is reported to the police while before leaving the accident scene.
Information required in the first notice of claims
- The name, address, and phone numbers of claimants and insureds and any witnesses
- The policy number of the insured
- The date and location of the accident, injury, loss or damage
- The details of the accident, injury, loss or damage
- Physicians name, address, phone and dates of treatment
